Unauthorized insurance policy opened using my Medicaid account

I got a notice about a final expense insurance policy for $10,000. They said the monthly premium was $356. I never authorized this at any time, not once.

I think the scammer got into my Aetna Medicaid account and used my information to set this up. They did cancel the policy and sent a $472 refund to my account, but that's not the full amount they took. I'm out $300 total from this. I never asked for any of this.
