Mail about form 5512 threatening jail time if I don't give info

I got some form 5512 document in the mail from the Annual Records Service. It was asking for my banking info, my SSN, all that stuff. Then it said if I don't provide this information I could go to jail for 2 years and pay a $500 daily fine until I give it to them.

Something felt off so I looked up the form name online. Turns out it's a common scam coming from that address they listed, 1030 15th St NW #366 in Washington DC. I shredded the letter before I thought to take a picture of it.

Got a mail form asking for money to Annual Records Service

I got something in the mail from Annual Records Service. It's a Form 5510 asking me to send a check or money order for $119.00, made payable to them. The return address is 1030 15th St NW #366, Washington, DC 20005. I never signed up for anything from them. This feels like a fake bill. I'm not paying it.

Got a letter demanding money for my LLC from an old address

I got this letter from something calling itself the Annual Records Service. It was addressed to my LLC but sent to an old address of mine, which seemed off right away.

They're saying I have to pay $119 for mandatory beneficial ownership reporting. The letter threatens hefty penalties and fines if I don't pay. They didn't give a phone number or email to contact them, only a P.O. Box address in Washington, DC. The only way to pay they listed is by check or money order sent to them.

It just doesn't feel right getting a demand for money like this, especially sent to an old address.

Got a letter in the mail demanding $119 for some state law report

I got this letter in the mail from the Annual Records Service. It came through USPS. It says I have to pay $119 to them because of some state law about reporting beneficial ownership information.

It wanted all my information—my name, address, EIN number, phone, and email. It even asked for a signature. And it told me to mail a check in this envelope they provided. The whole thing just felt off. I never signed up for anything with them.
