Strange Medicare billing for equipment I never ordered

I got a letter from Medicare saying they paid over $1700 for something I never got. It was billed for like 6 months last year, $2,250 total. My secondary insurance also got a bill and they denied it.

I called Medicare and reported it. But then it happened again. They paid that $1,764.00 four times. I don't know who this company is but the address they used is 1805 S Bellaire St 480-01 in Denver. I never ordered anything from them.

The phone number they gave is (720) 986-4960. I lost over $7,000 from this whole thing. I already told Medicare what happened but nothing seems to stop it.
