Charges for catheters I never ordered on my Medicare statement

I was looking at my Medicare statement and saw charges for intermittent urinary catheters on 4/10/25, 5/10/25, 6/10/25, and 10/10/25. Each charge was $2,250.00. I don't need catheters and I never asked for them. The doctor that ordered these is someone I don't even know.

Medicare paid $1,764.00 for each one. That's a lot of money. The company is called Centennial Med, their email is [email protected] and they have an address in Denver. I have no idea how they got my info. I just want to know where I go from here to report this.
