Job offer on Indeed turned into a weird tax form request

Someone reached out to me on Indeed about a job. They emailed me some info about the position and I did a phone interview where they explained the job description and details. Seemed normal enough at first.

Then they sent me forms to fill out for tax information. That just felt off. I didn't send anything back because it didn't feel right. The whole thing seems like a way to get my personal info. The phone number they used was (930) 207-8484 and the website is for-wardly.com with an address in Santa Clara.
