Got a weird final notice in the mail with a fake check

I got this letter in the mail saying it was a "FINAL NOTICE." It told me to call immediately to verify information for something called a Property Code and an address for a lender named Illinois Bk&Tr.

Inside was a check made out to me for $199.00. It was supposed to be for an "Allocated Waiver." The check didn't even have a signature on it. At the bottom of the letter it had a note saying "Not all consumers have previous coverage" and that it was "Not affiliated with Illinois Bk&Tr."

The whole thing felt off. It wanted me to call (888) 788-4779. I never signed up for anything from them.
