Billed for medical supplies I never got and don't even need

I got a bill from Medicare and my other insurance, UMR. They were charged around $7,000 for urinary tract supplies. I never got any supplies. I never ordered anything. I don't even need this stuff, I have no urinary tract issues at all.

I had to get a whole new Medicare number because of this. Why did Medicare and UMR pay this bill? There was no proof I needed or got anything. No money came out of my pocket, but it's just wrong. They paid $7,000 for nothing.
