Medical claim statements for supplies I never ordered from Memphis DME Supply

I got two medical claim statements from my health insurance company. They were for supplies I never ordered from Memphis DME Supply, LLC in Tennessee. Their address is 5118 Park Ave Ste 323, Memphis, TN 38117.

Somehow they got my health insurance card info and filed fake claims. I check all my claim statements carefully so I caught it. I'm sending another fraud claim to my insurance company. I'm also going to submit fraud claims other places to warn people.
