Someone filed fake health insurance claims after my info got stolen

I don't know what company to even put here because the people doing it didn't leave a name. But the address on the claims is 10711 RED RUN BLVD STE 101 in Owings Mills, MD 21117. They started filing a bunch of fraudulent healthcare claims with my insurance. Medicare/Medicaid type stuff.

My insurance info was stolen from NuMotion. That's where it came from. So now I'm getting all these fake claims and I didn't lose any money yet but it's a mess trying to get it sorted. No one will tell me who is actually behind it.
