Got a fake final notice letter about a house warranty

I got a letter today marked "Official Use." It had a fake Owner's ID on the form and said this was the final notice, giving a date to respond by. They even had the correct bank name for my mother, but she never had a house warranty like that.

I looked up the phone number on the letter, (888) 678-5131, on Safari. ScamPulse.com came up right away with information that it's a scam. The return address just says NM, USA. The whole thing felt off.
