Got a fake check from my own bank account

I just got a check in the mail that's completely bogus. It's drawn on my own bank account, made payable to some place called ProPlan Benefits. The account number on it is 99000036071.

The signature line on the check just says "Authorized By Customer." I never authorized any of this. The check came from phone number (855) 470-0304.

Nothing was taken out, but it's really unsettling to see a check like this. It just showed up out of nowhere.
