Getting threatening letters trying to scare me into a monthly plan

I got this letter that's trying to scare me. It looks official with a signature and an ID number on it, but it's not. It's from someone in Troy, OH. They're trying to pressure me into signing up for these really expensive monthly coverage plans.

The whole thing feels off. They even put a phone number, 1(888) 404-4504, and an email that's just [email protected]. It's clearly a trick to get people to respond. They didn't get any money from me, but they're sending out these phishing letters to try and trap someone. It's just a threatening letter to make you call them.
