Fake waiver check came with my name and address on it

Got this envelope in the mail from somewhere in Janesville, WI. It had a check inside for some kind of allocated waiver thing from US BNAS. They had my full name and home address on it, city state and zip all correct.

I didn't lose any money but it felt weird. It looked like a fake check or money order situation. No idea how they got my info but it was pretty specific to me. I just threw it away.
